Sorena Health LLC (“Sorena Health,” “we,” “us,” or “our”) operates sorenahealth.com and provides psychiatric and therapy services to patients in Arizona. This Privacy Policy explains what information we collect through our website and communications, how we use it, who we share it with, and the choices you have.
This policy covers information collected through our website, forms, phone calls, and text messages. Protected health information created or received in the course of treatment is governed separately by HIPAA and by our Notice of Privacy Practices.
INFORMATION WE COLLECT
Information you provide. Name, date of birth, phone number, email address, state of residence, the type of care you are seeking, insurance information including images of insurance cards, and any other information you submit through a form on our site or share with us by phone, text, or email.
Information collected automatically. IP address, browser and device type, pages viewed, time spent on the site, referring URL, and similar analytics information collected through cookies and comparable technologies.
Call and message records. We use third-party call tracking and messaging platforms to route, record, and log inbound and outbound calls and text messages for scheduling, quality assurance, training, and care coordination.
HOW WE USE INFORMATION
We use the information we collect to schedule, confirm, and remind you about appointments; verify insurance eligibility and benefits and process billing; respond to your questions and coordinate your care; operate, secure, and improve our website and services; and comply with our legal, regulatory, and contractual obligations.
SMS AND TEXT MESSAGING
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
You may opt out of text messages at any time by replying STOP to any message you receive from us. Opting out of text messages does not affect your ability to receive care, and we will continue to reach you by phone or email. Our full SMS terms are available on our Terms of Service page.
HOW WE SHARE INFORMATION
We share information with service providers who perform functions on our behalf, including our electronic health record vendor, telephony and call tracking providers, insurance clearinghouses and payers, and payment processors. Where these vendors handle protected health information, we require a Business Associate Agreement as HIPAA requires.
We may also disclose information when required by law, court order, or subpoena; to protect the rights, safety, or property of Sorena Health, our patients, or others; or in connection with a merger, acquisition, or sale of assets, subject to the protections in this policy.
We do not sell your personal information. We do not share your mobile number or SMS consent with anyone for marketing purposes.
COOKIES AND ANALYTICS

CHILDREN’S PRIVACY
Sorena Health treats patients ages four and older. Where a patient is a minor, we collect information from a parent or legal guardian and require their consent for treatment. Our website is not directed to children, and we do not knowingly collect information online directly from children under 13.
CHANGES TO THIS POLICY
We may update this Privacy Policy from time to time. The effective date at the top of this page reflects the most recent version. Material changes will be posted here.
