How referrals work
Send us the referral by phone, fax, or email and our team will reach out to the patient directly to schedule. We verify insurance coverage and ensure the patient knows what to expect at every step. Once care begins, with patient permission, we provide you ongoing updates on patient progress. Our goal is to support you and the patient, adding our valuable expertise to the care team.
What to send
Patient information
Name, date of birth, phone number, and the best time to reach them.
Reason for referral
A short summary of the concern and any treatment you have already tried.
Insurance details
Plan name and member ID if you have them. Our team verifies coverage before the first visit.
Relevant records
Recent notes, medication lists, or prior evaluations that would help our provider prepare.
